Job Summary:

We are seeking a skilled and compassionate Mental Health Practitioner to join our team at Hourglass. As a key member of our CAMHS team, you will play a vital role in providing high-quality care and support to young people with mental health needs.

Key Responsibilities:

  • Assessment and Support: Conduct thorough assessments of young people's mental health needs, identifying areas for support and developing personalized care plans.
  • Collaboration and Teamwork: Work closely with colleagues to ensure seamless care delivery, sharing knowledge and expertise to achieve the best outcomes.
  • Risk Management: Develop and implement risk management plans, adapting to changing circumstances and prioritizing the safety and well-being of young people.
  • Discharge Planning: Coordinate discharge planning, ensuring a smooth transition to community-based services and minimizing the risk of readmission.
  • Triage and Duty Support: Provide triage and duty support to the team, prioritizing caseloads and responding to urgent situations.
  • Signposting and Referrals: Signpost young people to appropriate services, making safeguarding referrals when necessary to ensure their safety and well-being.

Requirements:

  • Experience: Proven experience working with young people with mental health needs, including those with complex and high-risk presentations.
  • Skills: Excellent assessment and communication skills, with the ability to work independently and as part of a team.
  • Knowledge: Up-to-date knowledge of mental health legislation, policies, and procedures, including safeguarding and risk management.
