About the Role
Lancashire & South Cumbria NHS Foundation Trust is seeking an experienced Community Mental Health Practitioner to join our team. As a Community Mental Health Practitioner, you will play a vital role in providing high-quality care to individuals in the community.
Key Responsibilities
- Provide nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning, monitoring of care given to service users.
- Undertake mental health assessments, including risk assessments, within a multi-disciplinary assessment team.
- Refer to other clinicians within the service or refer to other services/agencies where necessary.
- Provide a fast, responsive service to assess individual's needs.

Requirements
- Registered Mental Health Nurse Degree/Diploma.
- On-going registration with the NMC.
- Specialist qualification or equivalent level in skills and development.
- Able to work as part of a multidisciplinary team.
- Experience of assessing, planning and implementing and reviewing care needs.
- Experience acting as a mentor/assessor of student staff.
- Can provide evidence of recent professional/personal development.
- Knowledge of common and complex mental health problems and medication used in relation to these.
- Clear ability to understand new concepts and ideas and excellent clinical practice.
- Understanding of Health and Safety policies and procedures.
- Knowledge and practice of Mental Health Act 1983.
- Demonstrate understanding of how research has influenced practice.
