Position Overview

We are pleased to announce an opportunity to become a part of our Clinical Nurse Evaluator Team within a prominent health organization.

We seek a candidate with exceptional evaluation and interpersonal skills. The ideal candidate will exhibit the capability to collaborate effectively within a team while also demonstrating the ability to operate independently across various organizations.

The role entails conducting evaluations for funding and care processes, necessitating a comprehensive understanding of the National Framework. In addition to assessing patients to confirm they receive appropriate care in suitable environments, the successful candidate will also oversee care quality standards within local care facilities and collaborate closely with other organizations, including local authorities and safeguarding teams.

Key Responsibilities

Clinical Nurse Evaluators are expected to:

- Conduct nursing evaluations, including Continuing Health Care assessments

- Review funding and care packages

- Possess knowledge of the updated CHC Framework

- Ensure quality standards of care in nursing facilities

- Collaborate closely with partner organizations, including local authorities, nursing facilities, care agencies, and safeguarding teams

- Identify care packages for clients in community settings

- Communicate with secondary care professionals

- Participate in Quality Assurance and CHC panels, presenting cases for approval

- Utilize Electronic Patient Record systems

- Mentor and educate colleagues and nursing students

Proficiency in the Welsh language is preferred; applications from both Welsh and English speakers are encouraged.
