About the Role

Central Cheshire Integrated Care Partnership (CCICP) is seeking a dedicated and passionate Community Diabetes Staff Nurse to join our dynamic team for a 6-month secondment. This role offers an exciting opportunity to contribute to the delivery of high-quality, evidence-based diabetes care within the community setting.

Your Responsibilities

  • Manage your own caseload of patients autonomously while collaborating effectively with a multidisciplinary team.
  • Empower patient choice and involvement by initiating appropriate actions under supervision.
  • Coordinate and develop comprehensive patient care pathways, liaising across various organizational boundaries.
  • Conduct specialized assessments and plan tailored programs of care, providing expert advice to patients, families, and caregivers.
  • Ensure each patient is placed on the appropriate treatment pathway at the right time, coordinating necessary investigations and interventions.
  • Maintain accurate patient records, documenting all relevant information in medical and nursing files.
  • Participate actively in multidisciplinary team meetings with consultants and specialists to discuss complex case management strategies.

Your Skills and Experience

  • Registered General Nurse (RGN) or Registered Nurse (RN) qualification.
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Exceptional communication and interpersonal skills.
  • Commitment to evidence-based practice.
  • Experience in diabetes care is highly desirable.
  • Previous experience managing a caseload is advantageous.
