We are seeking a highly skilled and experienced Virtual Ward Nurse Specialist to join our team at Mid Cheshire Hospitals NHS Foundation Trust. As a key member of our community care team, you will be responsible for the management and coordination of patients admitted to the Virtual Ward.
Key Responsibilities- Oversee the ongoing assessment of patient needs and develop, implement, and evaluate programmes of individualised care to patients as determined at Virtual Ward Multi-disciplinary Team meetings.
- Work as part of the integrated community health care team, collaborating with professionals and organisations across the 24-hour period of service delivery.
- Refer to and liaise with other services within the Central Cheshire Integrated Care Partnership when patient need requires it.
- Assist and support the Virtual Ward team leader in providing clinical leadership to the Virtual Ward team and leading the team in the provision of high-quality, evidence-based programmes of care.
- Qualified Nurse with a wide understanding and experience of community nursing.
- Excellent interpersonal and communication skills, written and verbal.
- Evidence of leading and managing staff and caseload management.
- UK driving licence and access to a car for work purposes.
