About the Role

We are seeking an experienced and skilled Advanced Clinical Practitioner Nurse to join our Primary Care Network team in North Lewisham. As a key member of our multidisciplinary healthcare team, you will play a vital role in delivering high-quality patient-centered care and contributing to the development of our network.

Key Responsibilities
  1. Clinical Care Delivery
  • Provide first-point-of-contact services for patients presenting with undifferentiated, undiagnosed problems, utilizing skills in history taking, physical examination, problem-solving, and clinical decision-making to establish diagnoses and management plans.
  • Make professionally autonomous decisions, taking accountability for actions.
  • Initiate necessary invasive and non-invasive diagnostic tests or investigations, interpreting findings and reports.
  • Prescribe safe, effective, and appropriate medication, adhering to current legislative frameworks.
  • Deliver safe, evidence-based, cost-effective, individualized patient care.
  • Offer holistic services to patients and their families, developing ongoing plans of care and support, emphasizing prevention and self-care.
  • Promote health and well-being through health promotion, health education, screening, and therapeutic communication skills.
  • Take a personalized care approach and population-centered care approach to enable shared decision-making with patients.
  • Refer patients directly to other services within or outside the network, and agencies as appropriate.
  • Participate in developing and delivering specialist primary care services, such as long-term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease, and hypertension).
  • Collaborate with nursing, medical, and healthcare assistant colleagues across the network to ensure National Service Frameworks (e.g., Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered.
  • Work with the network's multidisciplinary teams, as well as those within member GP practices and across the wider network, to promote integrated and seamless pathways of care.
  • Contribute to the network's achievement of quality targets to sustain high standards of patient care and service delivery.
  • Participate in identifying community health needs and developing patient/family-centered strategies to address them, in liaison with appropriate community health professionals and agencies.
  • Be aware of and contribute to planning and delivery of practice-based commissioning.
Professional Role
  • Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
  • Monitor the effectiveness of their own clinical practice through quality assurance strategies, such as audit and peer review.
  • Maintain professional registration.
  • Participate in continuing professional development opportunities to ensure up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.
  • Work within the latest NMC Code of Professional Conduct.
  • Record accurate consultation data in patient records, in accordance with the latest NMC guidance and other pertinent standards.
  • Keep up-to-date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.
  • Proactively promote the role of the Advanced Nurse Practitioner within the network and externally to key stakeholders and agencies.
  • Participate in multi-disciplinary protocol and patient group directions development.
  • Identify opportunities to delegate both clinical and administrative tasks to more junior staff.
  • Help the network operate in a cost-effective manner.
  • Identify and manage nursing care risks on a continuing basis.
  • Participate in network meetings.
  • Participate in audits and inspections as appropriate.
  • Work closely with clinical leads and administrative managers in setting up and/or improving systems for monitoring/measuring performance against indicators for the domains and areas outlined in the current IIF.
  • Work with member GP practices to assist them meet Clinical Governance and Quality Indicator targets.
  • Ensure that all network policies are fully implemented.
  • Work collaboratively with colleagues within and external to the network.
