Job Overview

The role of the practice nurse is integral to the provision of high-quality nursing services within our healthcare facility. As a member of the multidisciplinary team, you will deliver care tailored to the needs of our diverse patient population, ensuring that all services are provided within your professional scope.

Key Responsibilities

The practice nurse will undertake a variety of essential duties, which may include, but are not limited to:

  1. Design and implement health promotion initiatives aimed at enhancing patient well-being.
  2. Develop and assess individualized treatment plans for patients with chronic illnesses.
  3. Identify and support patients at risk of developing long-term health conditions.
  4. Administer routine nursing care in alignment with clinical guidelines and best practices.
  5. Provide specialized wound care and travel medicine services.
  6. Manage pathology requests and process results effectively.
  7. Conduct chronic disease management clinics and refer patients to specialists as necessary.
  8. Maintain comprehensive and accurate clinical records in compliance with legal requirements.
  9. Facilitate well woman and well man health clinics.
  10. Assist in minor surgical procedures under the supervision of GPs.
  11. Prioritize health concerns and respond appropriately to clinical emergencies.
  12. Implement vaccination programs for various age groups.
  13. Support patients in understanding their prescribed medications.
  14. Collaborate with external agencies to ensure comprehensive patient support.
  15. Uphold safeguarding standards in accordance with local and national policies.

Additional Responsibilities

In addition to the primary duties, the practice nurse may also be involved in:

  1. Leading clinical audits to enhance service delivery.
  2. Ensuring the safe management of medical supplies.
  3. Mentoring junior nursing staff and fostering a collaborative environment.
  4. Engaging in local health initiatives to improve patient care.
  5. Participating in continuous professional development and training.

Qualifications and Experience

Essential Qualifications:

  • Registered Nurse with a valid nursing license.

Desirable Qualifications:

  • Postgraduate qualification in Primary Care.

Required Experience:

  • Proven experience in a primary care setting, particularly in chronic disease management.

Preferred Experience:

  • Background in practice nursing or community health nursing.
