Job Overview
The role involves overseeing the provision of nursing services within a primary care setting, collaborating with a multidisciplinary team to deliver care to the designated patient population. The nurse will manage various clinical domains, including health promotion, chronic illness management, preventative health, and specialized clinics for both men and women, while also assisting the management team in evaluating clinical policies and procedures.
Key Responsibilities
The following are the primary duties expected of the clinical nurse. Additional tasks may arise based on workload and staffing conditions:
a. Design, implement, and integrate health promotion and wellness initiatives.
b. Develop and assess personalized treatment plans for patients with chronic conditions.
c. Identify and support patients at risk of developing long-term health issues, aiming to prevent negative health outcomes.
d. Deliver routine nursing care in line with clinical evidence, NICE guidelines, and national standards.
e. Provide wound care services, including ulcer and Doppler assessments.
f. Administer travel health services.
g. Request and manage pathology services as needed.
h. Process and interpret pathology results as required.
i. Conduct chronic disease management clinics, ensuring appropriate referrals to secondary or specialist care.
j. Maintain precise clinical documentation in accordance with current legislation.
k. Utilize read codes effectively for patient records.
l. Maintain registers for chronic diseases.
m. Establish and manage well woman clinics.
n. Establish and manage well man clinics.
o. Provide chaperone services when necessary.
p. Assist general practitioners with minor surgical procedures as needed.
q. Prioritize health concerns and intervene as appropriate.
r. Support the clinical team in emergency situations.
s. Recognize, evaluate, and refer patients with mental health needs.
t. Implement vaccination programs for both adults and children.
u. Assist patients in the proper use of prescribed and over-the-counter medications within the scope of practice.
v. Collaborate with external services to ensure comprehensive support for vulnerable patients.
w. Delegate clinical tasks appropriately, ensuring safe practices are followed.
x. Assist the clinical team with safeguarding issues, adhering to local and national policies.
y. Understand and apply local policies regarding substance abuse and addiction, referring patients as necessary.
z. Deliver opportunistic health promotion when suitable.
Additional Responsibilities
In addition to primary duties, the clinical nurse may be tasked with:
a. Leading audits and effectively utilizing the audit cycle.
b. Monitoring the safe storage, rotation, and disposal of medications.
c. Mentoring junior nursing staff and providing guidance as needed.
d. Engaging in local initiatives to improve service delivery and patient care.
e. Participating in shared learning experiences within the practice.
f. Continuously reviewing clinical practices in response to national policies and initiatives.
g. Participating in the evaluation of significant and near-miss events using structured approaches, including root cause analysis (RCA).
Qualifications and Experience
Essential Skills
- Proficient in wound care, ECGs, venepuncture, and new patient assessments.
- Capable of requesting and processing pathology tests and advising patients accordingly.
- Knowledgeable in travel medicine, chronic disease management, and immunizations.
- Understanding of evidence-based practice and clinical governance.
- Ability to maintain accurate clinical records and work within the scope of practice.
- Familiarity with health promotion strategies and the importance of PGDs.
Desirable Skills
- Awareness of public health issues in the local community.
- Understanding of broader health sector challenges.
