Health and Wellbeing Advisor

We are seeking a skilled Health and Wellbeing Advisor to join our team at Southdown, a leading provider of mental health services in Sussex.
About the Role:This is an exciting opportunity to work with clients with severe mental illness (SMI) to engage with their Physical Health Check outcomes and wider Social Prescribing support needs.
You will work in partnership with Sussex Partnership Foundation Trust and local GP's to provide targeted mental health and holistic support in Sussex.
Key Responsibilities:- Support clients to engage in their Physical Health Check outcomes following their annual Physical Health Check within their GP Surgery
- Work within the community and various sites in a peripatetic way
- Motivate clients to engage with and take ownership of their health and wellbeing
- Provide short-term support to a caseload of clients with complex needs
- Link clients into the community using a holistic approach to improve their health and wellbeing
- Promote and support the implementation and development of the service
- Identify areas of support within the community and mental health services that support the client's goals
To succeed in this role, you will need a good understanding of working within Mental Health and Wellbeing, Social Prescribing or equivalent transferable skills and a genuine passion for supporting people to explore their needs.
You will receive a comprehensive induction followed up with regular training - we will invest in you to give you all the tools you need to succeed.
You will join a supportive, flexible team and be working for one of the leading providers of mental health services in Sussex.
