Primary Care Mental Health Nurse Specialist

NHS Dumfries & Galloway's Primary Care Mental Health Service is seeking a skilled and dedicated Primary Care Mental Health Nurse to join their team on a permanent basis.
This is an exciting opportunity to work within Community Mental Health and gain experience in a primary care setting. The post holder will be aligned to Annandale and Eskdale Community Mental Health Team, but will be based within GP practices supporting Primary Care in the provision of early assessment and interventions for people with mild-moderate mental health issues.
The successful candidate will be responsible for supporting GP practices with triage, completion of mental health assessments, delivery of brief therapeutic interventions, and close partnership working with Primary Care, Home teams, Secondary Care Mental Health Services, and Third Sector Agencies.
Key responsibilities include:
- Supporting GP practices with triage and mental health assessments
- Delivering brief therapeutic interventions
- Participating in close partnership working with Primary Care, Home teams, Secondary Care Mental Health Services, and Third Sector Agencies
- Working in partnership with patients/carers and having good knowledge of local mental health support services
