Nurse - Physical Health Team

We are seeking a highly skilled and experienced Registered Nurse to join our Physical Health Team in Essex. As a Band 6 Registered Nurse, you will play a crucial role in supporting the delivery of physical health assessments to individuals with severe mental illness.
Key Responsibilities- Provide high-quality clinical support to junior colleagues and act as a clinical expert in accordance with evidence-based care and interventions.
- Contribute to the assessment of physical and mental health needs, with a focus on improving the physical health of individuals with severe mental illness.
- Ensure appropriate physical health observations and blood tests are taken and actioned for patients eligible for an annual physical health check.
- Assess patients holistically and provide advice on healthy lifestyles, signposting them to other services as required.
- Facilitate referrals to external services, including social services and secondary mental health services.

Requirements- Registered Nurse - Mental Health/Learning Disability/General, with current registration with the NMC.
- ENB 998/Preceptorship and or Practice Supervisor and Assessor Training.
- At least 3 years' experience post-registration at Band 5 and community nursing experience.
