Key Responsibilities
  • Provide clinical leadership and expertise in the management of physical health in individuals with severe mental illness.
  • Conduct comprehensive physical health assessments and develop care plans to promote healthy lifestyles and improve physical health outcomes.
  • Work collaboratively with primary care professionals, social services, and other stakeholders to ensure seamless care coordination and service delivery.
  • Support the development and implementation of service improvement initiatives to enhance the physical health and wellbeing of our service users.
  • Provide education and training to junior staff and students on physical health and wellbeing.

Requirements
  • Registered Nurse - Mental Health/Learning Disability/General with current registration with the NMC.
  • ENB 998 / Preceptorship and or Practice Supervisor and Assessor Training.
  • At least 3 years' experience post-registration at Band 5 and community nursing experience.
