We are seeking a highly organized and detail-oriented Patient Pathway Coordinator to join our Community Paediatric team at Oxford University Hospitals NHS Foundation Trust.

Key Responsibilities- Designated point of contact for consultants and clinic teams, ensuring consistency in administrative workflows.
- Support administrative team members, coordinate coverage during absences, and escalate issues when necessary.
- Proactively handle telephone inquiries from patients, carers, and other stakeholders, providing accurate and timely information.
- Manage patient follow-up appointments and administrative tasks to track patients effectively, ensuring timely and efficient care.
- Oversee clinic appointment bookings through the Choose and Book system, ensuring seamless coordination and minimal delays.
- Handle correspondence, ensure urgent matters are addressed promptly, and maintain patient confidentiality, adhering to data protection regulations.
- Transcribe clinic letters, reports, etc., and ensure timely dispatch, maintaining accurate and up-to-date records.
- Process patient referrals efficiently and according to Trust processes, ensuring compliance and minimal delays.
- Check future clinics, coordinate with the medical team, and address booking issues, ensuring seamless coordination and minimal disruptions.
- Obtain necessary diagnostic tests/hospital notes for clinics as required, ensuring timely and efficient care.
- Resolve complaints from patients and escalate concerns as needed, maintaining a professional and respectful demeanor.
- Essential: Administration experience and strong organizational skills.
- Desirable: Knowledge of electronic patient record systems and access standards.
