About the Role

We are seeking a highly skilled and compassionate Clinical Nurse Specialist to join our team at St Columba's Hospice. As a key member of our community hospice team, you will play a vital role in delivering exceptional palliative care to patients and their families.

Key Responsibilities:

  • Collaborate with healthcare professionals to provide holistic care to patients with palliative needs
  • Develop and implement individualized care plans to address symptoms and improve quality of life
  • Work closely with the multidisciplinary team to ensure seamless care delivery
  • Provide education and support to patients, families, and caregivers
  • Contribute to the development and implementation of hospice care policies and procedures

Requirements:

  • Registered Nurse with experience in palliative care
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Compassionate and empathetic approach to patient care
  • Knowledge of palliative care principles and practices
