About the Role

We are seeking a compassionate and experienced Registered Nurse to join our Hospice Team. As a Palliative Care Staff Nurse, you will be responsible for providing holistic care to patients and their families, working closely with a multi-professional team to ensure the best possible outcomes.

Key Responsibilities
  • Initial and ongoing assessment of patients' needs, planning and delivery of care to meet their practical, psychosocial, and spiritual needs
  • Responding to queries and requests for advice and support from patients, families, and colleagues
  • Working collaboratively with the Access Team and community setting to provide responsive and reactive care
  • Participating in the development and implementation of care plans, ensuring patient-centered and person-centered approaches
Requirements
  • Registered Nurse with palliative care experience
  • Excellent interpersonal and communication skills
  • Ability to work as part of a multi-professional team
  • Proven ability to work in a fast-paced environment, prioritizing tasks and managing workload effectively
