Job Title: Mental Health Practitioner

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at Cheshire and Wirral Partnership NHS Foundation Trust.

About the Role

This is an exciting opportunity to work in a newly established service that provides specialist assessment, treatment, interventions, and support to help individuals recover from complex mental health problems and regain the skills and confidence to live successfully in the community.

Main Responsibilities
  • Work intensively with a small cohort of patients with complex needs, providing high-quality levels of care to assist them in living independent lives.
  • Collaborate with the multidisciplinary team to ensure effective and adaptive patient care, with a strong focus on recovery, identifying relapse indicators, and promoting independence.
  • Develop and implement bespoke support packages to meet the unique needs of each patient.
  • Conduct professional/clinical supervision and provide specialized advice in relation to care.
  • Work closely with commissioners and care providers to ensure seamless care pathways.
Requirements
  • Educated to diploma/degree level and registered with the appropriate professional body.
  • Professional knowledge acquired through a relevant degree plus relevant experience for working with complex cases.
  • Evidence of continued professional development.
  • Relevant mentorship qualification as required by governing body.
