Job Title: Mental Health Practitioner

Grosvenor Hart Homes is seeking a highly skilled Mental Health Practitioner to join our team. As a Mental Health Practitioner, you will play a vital role in providing high-quality mental health assessments and support to children, young people, and families who are GHH tenants or applicants.

Key Responsibilities:
  • Conduct thorough mental health assessments to identify the needs of GHH tenants and applicants.
  • Develop and implement personalized support plans to address mental health needs.
  • Work closely with GHH's Family Assistant Service to ensure seamless support for tenants.
  • Build and maintain relationships with local primary care providers and community wellbeing services.
  • Coordinate referrals to specialist mental health providers and ensure timely access to services.
  • Provide non-clinical, low-level mental health and wellbeing support to prevent escalation of conditions.
  • Participate in supervision and regular team meetings to ensure effective collaboration and communication.
Requirements:
  • Registered Mental Health Nurse (minimum Band 7, maximum Band 8a)
  • Qualified and regulated to complete assessments for ADHD/ASD
  • Experience of working with a wide range of mental health conditions
  • Recent experience of working in a clinical setting, preferably delivering Children and Young People's Mental Health services
  • Excellent interpersonal skills, able to build trusted relationships and develop compassionate, empathetic, and supportive relationships with tenants
  • Outstanding problem-solving skills
  • Good and clear communication skills (written and oral)
  • Reliable with strong work ethic
  • Attention to detail and dedication to providing quality, error-free work
  • Professionalism, discretion, and maintaining confidentiality at all times
Working Arrangements:

This role requires occasional evening and weekend work, as well as occasional travel within the northwest of England and London. A full driver's license and access to a car would be desirable. This post holder will be required to work with vulnerable people in their own homes and will require an enhanced DBS check.
