Join Our Team as a Mental Health Practitioner

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at Cheshire and Wirral Partnership (CWP). As a Mental Health Practitioner, you will play a vital role in providing high-quality care to individuals with complex mental health needs.

About the Role

This is an exciting opportunity to be part of a newly established Mental Health Intensive Support Service, which provides specialist assessment, treatment, and interventions to help individuals recover from complex mental health problems.

As a Mental Health Practitioner, you will work intensively with a small cohort of patients, providing high-quality levels of care to assist them in living independent lives. You will work closely with the multidisciplinary team to ensure effective and adaptive patient care, with a strong focus on recovery, identifying relapse indicators, and promoting independence.

Key Responsibilities
  • Provide high-quality care to individuals with complex mental health needs
  • Work intensively with a small cohort of patients to assist them in living independent lives
  • Develop and implement individualized care plans to promote recovery and independence
  • Collaborate with the multidisciplinary team to ensure effective and adaptive patient care
  • Identify relapse indicators and develop strategies to prevent hospital admission
Requirements

To be successful in this role, you will need:

  • A relevant degree in a field such as mental health, social work, or a related field
  • At least 2 years of post-registration experience working with adults with mental health problems
  • Experience of working within a recovery-focused philosophy framework
  • Knowledge of evidence-based interventions and their application in a community setting
  • Excellent communication and interpersonal skills
