This expanded service will function across the integrated pathway from acute to intermediate care services in the community, providing essential support for patients in their care and rehabilitation journey, thereby improving clinical outcomes and patient satisfaction. We are committed to investing in both therapy and care roles, and successful candidates will have the opportunity to join a dynamic, forward-thinking, integrated, multi-agency, and multidisciplinary team dedicated to delivering comprehensive care to the residents of East Sussex.
As a valued team member, you will work closely with and receive guidance from our Professional Leads, Advanced Clinical Practitioners, Team Leads, Practice Educators, and the wider team, allowing you to enhance your skills, training, education, and career development, whether you are newly qualified or aspiring to a senior role.

Key Responsibilities
This position involves community-based responsibilities; therefore, you must be able to travel efficiently throughout the area, carrying necessary equipment and staff. A car and a valid driving license are essential.

Job Qualifications
Essential
- Good level of general education, including English and Maths
- National Vocational Qualification (NVQ) Level 3 or equivalent practical experience
- Satisfactory General Education and evidence of suitable additional training related to the role
Desirable
- Educated to GCSE level
Work Experience
Essential
- Prior experience in nursing or social care settings
- Customer service experience
- Experience working independently and as part of a team
Desirable
- Previous experience in community nursing and/or Physiotherapy, Occupational Therapy
- Experience working in a community setting
Knowledge Requirements
Essential
- Fundamental understanding of the NHS and healthcare system
- Knowledge of basic infection control principles
