About the Role

We are seeking a compassionate and empathetic individual to join our Home Treatment Team (HTT) NW as a Care and Community Services Officer. This is a fantastic opportunity to support our team in the community and make a real difference in people's lives.

As a Care and Community Services Officer, you will work closely with our senior support workers, qualified nurses, Clinical Team Leads, and medical team to provide holistic care to our clients. This includes physical health checks, identifying social stressors, and signposting patients to relevant services.

You will be responsible for communicating with clients, their carers, and other professionals to maximise care, promote crisis resolution, and minimise risk. If you have a passion for supporting individuals in their own home environment and have a full and valid driving licence, we would love to hear from you.

Surrey and Borders Partnership NHS Foundation Trust is the leading provider of health and social care services for people of all ages with mental ill-health and learning disabilities in Surrey and North East Hampshire. We also provide social care services for people with a learning disability in Croydon and ASD and ADHD assessment services in Hampshire.

Person Specification
  • Ability and desire to obtain underpinning knowledge and NVQ qualifications
  • Commitment to undertake Continuing Personal Development
Experience
  • Experience of mental health services (as a worker, service user, or carer)
  • Previous experience as a support worker
