Job Summary:

We are seeking a highly skilled and motivated Reablement Liaison Officer to join our team at Norfolk. As a key member of our social care team, you will be responsible for providing excellent customer service and ICT skills to support individuals in need.

Key Responsibilities:

  • Take referrals into our service via phone and Liquid Logic, our social care system.
  • Set up visits to service users for support workers using electronic scheduling software.
  • Provide information and support to GPs, community health services, social services, and service users and their families.
  • Take referrals for Norfolk First Support, a service that delivers support to adults in their own homes.

Requirements:

  • Excellent ICT skills and ability to work to tight deadlines.
  • Ability to assess risk and take appropriate action.
  • Excellent customer service skills and ability to work independently.
  • Ability to work as part of a multidisciplinary team to support individuals to return to their homes.

Working Arrangements:

The role is hybrid, with a mix of working from home and a locality office. The rota is on a two-week basis, with a mix of morning and evening shifts, alternative weekends, and bank holidays.
