Position Overview

The Patient Pathway Administrative Coordinator will play a vital role within the Community Paediatric administrative team, focusing on the coordination of patient care pathways.

Working Hours

This is a full-time position comprising four days from 08:00 to 16:00, alongside one late shift from 09:00 to 17:00.

Role Responsibilities

The individual in this role will serve as the primary contact for all administrative matters concerning the management of patient care pathways. Collaborating with a multi-disciplinary team, the coordinator will facilitate the effective management of patient journeys from initial referral through to discharge, ensuring a seamless experience for both patients and healthcare professionals.

Key responsibilities include:

  1. Processing and documenting all referrals in accordance with established policies and procedures.
  2. Scheduling new and follow-up appointments, optimizing clinic capacity to ensure patients are seen by the most suitable clinician promptly.
  3. Coordinating necessary diagnostics and investigations, ensuring results are available for consultations.
  4. Managing appointments and procedures across various healthcare facilities as required.
  5. Maintaining accurate records and ensuring timely communication with all stakeholders involved in the patient pathway.
  6. Monitoring and managing outpatient and theatre scheduling to maximize resource utilization.
  7. Providing exceptional customer service and acting as a reliable point of contact for patients and their families.

Qualifications and Skills

Essential qualifications include:
