Job Overview

We are seeking a highly skilled and experienced District Nurse Caseload Manager to join our Unplanned Care Team. As a key member of our team, you will be responsible for overseeing the provision of high-quality, holistic care for patients in their own homes with acute, complex, and long-term conditions.

Main Responsibilities
  • Lead a team of qualified nurses, nursing associates, and healthcare support workers to deliver high-quality patient care.
  • Develop and implement effective care plans to ensure timely and appropriate assessment and treatment of patients.
  • Collaborate with multidisciplinary teams to achieve the best outcomes for patients.
  • Ensure that team members are appropriately trained, supported, and equipped with the necessary knowledge, skills, and competencies to meet the requirements of the role.

Person Specification
  • Current NMC Registration or relevant allied healthcare professional qualification.
  • Experience of care coordination/care management.
  • Awareness of current Health and Social Care policies.
