Main Duties- Lead a caseload of patients with complex and long-term conditions, providing high-quality holistic care and ensuring timely interventions to prevent unnecessary hospital admissions.
- Develop and implement care plans in collaboration with patients, carers, and other healthcare professionals.
- Provide clinical leadership and guidance to a team of qualified nurses, nursing associates, and healthcare support workers.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.
- Current NMC registration as a District Nurse.
- Experience of care coordination and care management.
- Knowledge of current health and social care policies.
- Ability to work autonomously and as part of a team.
- Excellent communication and interpersonal skills.
