Job Summary

We are seeking a highly skilled and experienced Senior Community Nurse to join our dynamic team at Bradford District Care NHS Foundation Trust. As a Senior Community Nurse, you will play a key role in delivering high-quality patient care and leading a team of community nurses.

Main Responsibilities
  • Assess, plan, implement, and evaluate patient care in the community setting.
  • Lead a team of community nurses and provide guidance and support to ensure the delivery of high-quality patient care.
  • Develop and maintain effective relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.
  • Contribute to the development and implementation of clinical policies and procedures to improve patient outcomes.
  • Participate in quality improvement initiatives and contribute to the development of new services and programs.

Person Specification

We are looking for a Senior Community Nurse who has:

1st Level Registered Nurse qualification.Postgraduate qualification in a specialist field or equivalent experience.Driving license.Excellent communication and interpersonal skills.Ability to work autonomously and as part of a team.Experience of leading and managing a team of healthcare professionals.Knowledge of clinical governance and quality improvement principles.
