Job Title: Community Staff Nurse

We are seeking a highly motivated and caring Community Staff Nurse to join our dynamic team in Unplanned Care. As a key member of our team, you will be responsible for delivering high-quality healthcare to patients in their own homes, working collaboratively with other disciplines to achieve the best outcomes.

About the Role

This is an exciting opportunity to work in a fast-paced and innovative environment, where you will be part of a seven-day service covering 24 hours a day. You will be working with a variety of patients, assessing and managing their health needs, preventing hospital admissions, and facilitating early discharges.

Key Responsibilities
  • Work as part of the community nursing team in unplanned care, ensuring high-quality healthcare is delivered to patients in their own homes.
  • Participate in the assessment of health needs, delivery of care to the practice population, preventing hospital admissions, and facilitating early discharges from hospital.
  • Work collaboratively with other disciplines, including community partnerships and primary care networks.
