About the Role

We are seeking a highly motivated and caring Community Health Nurse to join our dynamic team in Unplanned Care. As a key member of our team, you will be responsible for delivering high-quality healthcare to patients in their own homes, working collaboratively with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients.

Main Responsibilities

  • To work as part of the community nursing team in unplanned care, ensuring high-quality healthcare is delivered to patients in their own homes.
  • To participate in the assessment of health needs, delivery of care to the practice population, preventing hospital admissions, and facilitating early discharges from hospital.
  • To work in collaboration with the wider Community Partnerships and Primary Care Networks.
