Community Nurse

We are seeking a highly motivated and caring Community Staff Nurse to join our dynamic team in Unplanned Care. As a key member of our team, you will be responsible for delivering high-quality healthcare to patients in their own homes, working collaboratively with other disciplines to achieve the best outcomes.
Key Responsibilities- Work as part of the community nursing team in unplanned care, ensuring high-quality healthcare is delivered to patients in their own homes.
- Participate in the assessment of health needs, delivery of care to the practice population, preventing hospital admissions, and facilitating early discharges from hospital.
- Work in collaboration with the wider Community Partnerships and Primary Care Networks.

Requirements- RGN qualification and current NMC registration.
