Job Summary

We are seeking a highly skilled and experienced Senior Community Nurse to join our dynamic team. As a Senior Community Nurse, you will be responsible for providing high-quality patient care in a community setting, working closely with patients, carers, and other healthcare professionals to achieve the best possible outcomes.

Main Responsibilities
  • Assess, plan, implement, and evaluate patient care in a community setting
  • Develop and maintain effective relationships with patients, carers, and other healthcare professionals
  • Work collaboratively with the multidisciplinary team to achieve shared goals and objectives
  • Contribute to the development and implementation of care plans and policies
  • Provide education and support to patients, carers, and other healthcare professionals

Person Specification

We are looking for a highly motivated and experienced Senior Community Nurse who possesses the following skills and qualifications:

  • 1st Level Registered Nurse qualification
  • Postgraduate qualification in a specialist field or equivalent experience
  • Driving license
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
