As a key member of our Unplanned Care Team, you will play a vital role in delivering high-quality patient care and supporting the clinical leadership of the team.
Key Responsibilities- Assess, plan, implement, and evaluate patient care in a community setting
- Develop and maintain effective relationships with patients, carers, and multidisciplinary teams
- Support the clinical leadership of the team and contribute to the development of clinical policies and procedures
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives and contribute to the development of best practices in community care
- 1st Level Registered Nurse qualification
- Postgraduate qualification in a specialist field or equivalent experience
- Driving license
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
