We are seeking a highly skilled and experienced District Nurse Caseload Manager to join our Unplanned Care Team. As a key member of our team, you will be responsible for leading a team of qualified nurses, nursing associates, and healthcare support workers in providing high-quality, patient-centered care to individuals in their own homes.
Main Duties and Responsibilities- Lead a team of healthcare professionals in providing clinical care to patients with acute, complex, and long-term conditions.
- Develop and implement care plans that meet the individual needs of patients, ensuring timely and effective delivery of care.
- Collaborate with multidisciplinary teams to achieve the best outcomes for patients, including hospital admissions and discharges.
- Ensure that team members are appropriately trained, supported, and equipped to meet the requirements of the role.
- Work closely with patients, carers, and families to ensure that their needs are met and that they receive high-quality care.
- Current NMC registration as a District Nurse or relevant allied healthcare professional qualification.
- Experience of care coordination and care management.
- Knowledge of current Health and Social Care policies.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Excellent communication and interpersonal skills.
