About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Lincoln. As a Support Worker, you will play a vital role in delivering outreach support to individuals with mental health conditions, empowering them to live the lives they choose.

Key Responsibilities
  • Assist with personal care and hygiene, medication administration, and support with daily living tasks.
  • Support individuals with preparing and cooking meals, attending medical appointments, and engaging in social activities.
  • Keep accurate records and update care plans as required, ensuring the highest standard of care.
  • Participate in training and development opportunities to enhance your skills and knowledge.
Requirements
  • Full UK/International driving license and access to a vehicle, with mileage paid at 0.45p per mile.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Ability to follow a Support Plan and Risk Assessments, and maintain accurate customer records.
  • Willingness to achieve the Care Certificate and commit to ongoing professional development.
