About the Role

We are seeking a skilled and compassionate Community Nurse to join our Felixstowe Community Healthcare Team. As a Community Nurse, you will play a vital role in providing high-quality nursing care to housebound residents in Felixstowe and surrounding areas.

Key Responsibilities
  • Triage, assess, plan, implement, and evaluate all aspects of nursing patient care, developing individualized care plans as needed.
  • Ensure informed consent is obtained and documented prior to initiating interventions.
  • Conduct holistic assessments, devising evidence-based care plans for each patient, modifying them as required.
  • Communicate complex and sensitive information to patients and carers, using tactful and persuasive skills to gain cooperation.
  • Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care coordinated.
  • Maintain skills with regards to latest technology and participate in multi-disciplinary meetings as appropriate.
  • Ensure own workload is planned and prioritized according to patient needs, completing patient-held records and SystmOne inputting daily.

Requirements
  • Current NMC registration
  • RN Qualification (Part 1 of register) or equivalent
  • Car Driver and access to a car for work, which is community-based

Desirable qualifications include Leg Ulcer Management, IV Administration/Cannulation, ENB 998/Mentorship Preparation, and Mentorship Qualification. Previous experience working in the community is also desirable.
