About the Role

We are seeking a skilled and compassionate Community Health Nurse to join our team at East Suffolk and North Essex NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.

Key Responsibilities
  • Assess and plan individualized care plans for patients with complex needs, including long-term neurological disorders, palliative and end-of-life care, diabetes, continence, and wound care.
  • Develop and implement care plans that promote patient independence and well-being, while also preventing hospital admissions and supporting early discharge.
  • Liaise with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Conduct regular home visits to assess patient needs, provide education and support, and make adjustments to care plans as necessary.
  • Collaborate with multidisciplinary teams to develop and implement care strategies that address the physical, mental, and social health needs of patients.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Current NMC registration as a Registered Nurse (Part 1 of the register).
  • Valid car driver's license and access to a car for work-related purposes.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage a caseload.
  • Ability to work autonomously and as part of a team, with a commitment to delivering high-quality patient care.
