About the Role

Haven Health, a member of Suffolk Primary Care, is on the lookout for a dedicated and empathetic Medication Management Specialist to become part of our vibrant, multi-disciplinary team. This position is available on a full-time basis, although part-time arrangements may be considered for the ideal candidate. Our organization is committed to enhancing the quality and safety of care provided to our patients.

As a Medication Management Specialist, you will play a crucial role in:

  • Patient Interaction: Engage in patient-facing roles to facilitate effective medication use through collaborative decision-making.
  • Medication Optimization: Execute tasks related to medication optimization, ensuring safe and effective administration of medications.
  • Medication Reviews: Assist in conducting medication reviews and reconciling medications for new patients, coordinating with local community pharmacists.
  • Public Health Support: Provide specialized knowledge to address public health and social care needs, offering lifestyle advice and service information.
  • Clinical Liaison: Collaborate with specialist pharmacists to manage complex patient cases within shared care protocols.
  • Antimicrobial Stewardship: Contribute to initiatives aimed at promoting responsible antibiotic prescribing and implementing national prescribing guidelines.
  • Care Navigation: Guide practice care navigation to streamline general prescription requests, allowing clinical staff to focus on more complex cases.
  • Leadership in Medication Systems: Lead efforts in optimizing medication systems across practices, promoting electronic prescriptions and safe repeat prescribing.
  • Training and Support: Provide training on the legal and secure handling of medications.
  • Additional Duties: Undertake other responsibilities as agreed upon with your line manager.
About the Candidate

The ideal candidate will possess:

  • A registration as a pharmacy technician with the GPhC.
  • A Diploma Level 3 in pharmacy services or an equivalent qualification.
  • Extensive experience in either a community pharmacy or primary care environment.
  • Confidence in analyzing and presenting information to healthcare professionals.
  • A detail-oriented approach with strong communication skills, both written and verbal.
  • Proficiency in computer skills, including MS Office.
  • The ability to interpret complex clinical information and present it effectively to diverse audiences.
