Role Overview

As a Lead Worker in Intensive Case Management, you will engage with members in a psychologically informed manner, establishing open, trusting, and high-quality professional relationships. You will assist individuals experiencing homelessness who face multiple and complex challenges, including but not limited to mental and physical health issues, substance use disorders, social exclusion, and isolation. Your role will involve conducting comprehensive assessments of each member's needs and strengths, ensuring they receive tailored support to overcome their homelessness. You will adopt a holistic perspective on each member's journey, adhering to our 'one worker, one member' approach.

Candidate Profile

We seek a skilled and motivated individual to join our Intensive Case Management team. The ideal candidate will have a proven track record of providing flexible, person-centered support to individuals. You will become part of a highly experienced and supportive team dedicated to enhancing the quality of life for those experiencing homelessness.
