Job Summary:

We are seeking a highly motivated and compassionate Support Worker to join our Community Re-ablement Team. As a Support Worker, you will play a vital role in supporting individuals with disabilities and older people to regain their independence and live fulfilling lives.

Key Responsibilities:

  • Provide person-centered support to individuals with disabilities and older people in their own homes or community settings.
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores.
  • Support individuals to access community resources and services, such as healthcare, education, and leisure activities.
  • Work collaboratively with other healthcare professionals and community agencies to ensure seamless care and support.
  • Participate in regular team meetings and training sessions to enhance skills and knowledge.

Requirements:

  • Hold a UK driving license or intend to gain one.
  • Be able to work flexibly, including evenings and weekends.
  • Have excellent communication and interpersonal skills.
  • Be able to work independently and as part of a team.
  • Have a passion for supporting individuals with disabilities and older people.
