Job Overview

We are seeking a skilled professional to join our Forensic Outreach Liaison Service as a Link Practitioner. This 12-month pilot role will work closely with our Community Mental Health Teams (CMHT) in Kent to ensure seamless discharge planning and transfer of care.

Main Responsibilities

  • Identify and address issues related to patient care and discharge barriers between our FOLS team and locality teams.
  • Collaborate with patients to support their transition from secure services to the community.
  • Communicate effectively with service users, some of whom may display challenging behaviors.
  • Develop strong relationships with stakeholders in criminal justice, statutory, and third sector organizations.
  • Provide specialist clinical advice to multi-agency teams and support service users in the community.
  • Work collaboratively with managers and clinical leads to improve the quality of care.
  • Ensure practice is evidence-based and responsive to individual service users' needs.
  • Educate staff on national and local policy and professional guidelines.
  • Escalate issues of poor professional conduct or practice as necessary.
