About the Role

We are seeking a highly motivated and organized Care Coordinator to join our homecare team at Nurseplus. As a Care Coordinator, you will be the first point of contact for both new and existing service users and care workers, handling initial enquiries and providing support to ensure the delivery of high-quality care.

Key Responsibilities

  • Manage and coordinate the scheduling of care workers to meet the needs of service users.
  • Develop and maintain strong relationships with service users, their families, and care workers to ensure a seamless delivery of care.
  • Monitor and report on the quality of care provided, identifying areas for improvement and implementing changes as needed.
  • Collaborate with other healthcare professionals to ensure comprehensive care planning and coordination.
  • Recruit and onboard new care workers, providing training and support to ensure they are equipped to provide high-quality care.
  • Conduct regular supervisions and appraisals to ensure care workers are meeting the required standards.
  • Manage and resolve any incidents, complaints, or safeguarding concerns in a timely and professional manner.

Requirements

  • A full UK driving license is essential for this role.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, care workers, and other healthcare professionals.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • A flexible and adaptable approach, with the ability to work in a fast-paced environment and respond to changing demands.
  • A positive and customer-focused attitude, with a commitment to delivering high-quality care and excellent customer service.
