We are seeking to appoint a motivated and adaptable Band 5 nurse to enhance the Intravenous Therapy (IV) at Home Service throughout the Cheshire East region. This role is designed for a Band 5 Adult Nurse who is eager to engage with a progressive workforce, available for both full-time and part-time commitments within a 7-day service framework.

Key Responsibilities

The successful candidate will be required to complete a comprehensive training program focused on the management and care of Mid and PICC lines, cannulation, Aseptic Non-Touch Technique (ANTT), and the administration of IV medications. Proficiency in clinical risk assessment will be crucial for delivering safe, high-quality care.

The candidate must be a registered nurse, showcasing effective communication and leadership abilities, and must be capable of working autonomously while also being an integral part of a Multidisciplinary Team (MDT) to ensure a secure patient journey. A commitment to ongoing learning and professional development is essential.

Essential Duties

  • Operate within the established framework for Community Nursing Services.
  • Assess, deliver, and review comprehensive nursing care, guidance, and information to patients and caregivers within their home or suitable care settings, ensuring safety and professionalism through evidence-based practices.
  • Conduct initial assessments upon receiving specific referrals, recognizing the needs of clients and their caregivers.
  • Plan, implement, and evaluate care strategies to address the physical, social, emotional, and environmental needs of patients, incorporating a rehabilitative/self-care approach as necessary.
  • Provide skilled nursing care and perform clinical tasks, including the administration of medications via intravenous lines, venepuncture, cannulation, and wound management.
  • Maintain awareness of legislation regarding IV Therapy and associated devices, delivering safe care and advice within the community context.
  • Ensure the clinical environment is safe for practice by maintaining cleanliness and readiness of materials and equipment.
  • Demonstrate, instruct, and supervise nursing care that can be safely delegated to patients or caregivers, providing support as needed.
  • Promote clinical excellence and an evidence-based approach to care by adhering to Trust policies and procedures and maintaining clinical competencies.
  • Engage in health education for individuals and the community, advocating for self-care and providing both planned and opportunistic care based on evidence-based information.
  • Accurately document all patient interactions using Trust documentation, ensuring confidentiality in accordance with NMC guidelines.
  • Ensure that necessary equipment and aids for effective care are maintained and demonstrate safe usage to patients and/or caregivers as required.
  • Participate in the delivery of care for patients with complex needs, offering support to caregivers and relatives in challenging situations.
  • Refer individuals to other practitioners when needs exceed one's scope of practice or require long-term support.
  • Effectively manage and prioritize your caseload.
  • Contribute to the measurement and monitoring of quality standards as appropriate.
  • Maintain accurate personal and professional records in line with Trust and Nursing and Midwifery Council policies.
  • Utilize all resources (human, physical, and financial) efficiently and cost-effectively.
  • Report any accidents or incidents involving patients or self while on duty and complete the necessary documentation promptly.
  • Collaborate with colleagues to coordinate off-duty and holiday leave, ensuring service continuity.
  • Adhere to MCHFT Integrated Governance Strategy and requirements, actively participating in appraisal and clinical supervision as per Trust policy.
  • Act with respect for privacy, dignity, and confidentiality at all times.
  • Promote the individual needs of patients by acknowledging their preferences, rights, and choices, and by fostering anti-discriminatory practices.
