An exceptional opportunity has emerged for Registered Nurses to engage with the Community Continence and Urology Service on a full-time permanent basis.
We seek passionate individuals who are keen on the fields of continence and urology. You will be an integral part of a well-established team dedicated to providing specialized care across various community settings and care facilities.
The primary focus of this role is to conduct continence assessments and to design, implement, and evaluate specialized continence care programs. The successful candidate will also be involved in training patients, their caregivers, colleagues, and the broader Health/Social Care community.

The Community Continence & Urology Service delivers specialized care to patients, their families, caregivers, and colleagues throughout the West Cheshire area. Patients are seen in community settings, care homes, and clinical environments.
The successful candidate will become part of a dedicated team of clinicians, nurses, and administrative staff.
Main Responsibilities1. Conduct continence assessments and develop, implement, and evaluate treatment plans for patients.
2. Maintain professional responsibility for your own caseload.
3. Participate in nurse-led continence clinics.
4. Execute clinical skills pertinent to the role.
5. Provide training to caregivers.
6. Ensure confidentiality in managing your caseload notes.
7. Collaborate with external agencies to enhance the visibility of continence promotion.
8. Communicate effectively with relatives, clients, agencies, and the multidisciplinary team.
9. Act as an advocate for clients at all times.
10. Develop communication strategies to ensure understanding of care programs and adherence to treatment plans.
11. Foster positive relationships with other statutory and voluntary services through collaborative efforts.
12. Maintain effective communication within the continence team.
13. Communicate sensitive information regarding financial, sexual, and interpersonal issues with care.
14. Manage and prioritize your own caseload effectively.
15. Operate within the budget of the continence service.
16. Engage in risk assessments for patients, caregivers, and staff.
17. Provide guidance and information to staff as needed.
18. Participate in the clinical audit process in collaboration with the audit department as appropriate.
