Healthcare Assistant

We are seeking a compassionate and skilled Healthcare Assistant to join our multidisciplinary community-based team at Derbyshire and South Staffordshire. As a key member of our team, you will provide holistic, person-centred care to patients and their loved ones, supporting them through the end-of-life journey.
Key Responsibilities- Deliver high-quality, person-centred care and support to patients and their families, prioritizing their physical, emotional, and spiritual needs.
- Develop and implement care plans in collaboration with other healthcare professionals, ensuring the complex needs of patients are met.
- Provide empathetic communication and support to patients and their loved ones, promoting a positive and respectful care environment.
- Maintain accurate records and reports, adhering to regulatory requirements and national professional standards.
- National Vocational Qualification (NVQ) Level 3 in Health and Social Care or equivalent alternative qualification.
- Minimum of six months' recent care delivery experience in a Health or Social Care Environment, providing care and support to dying patients.
- Proven ability to work effectively in a multidisciplinary team, prioritizing patient needs and collaborating with other healthcare professionals.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
