About the Role

We are seeking a highly motivated and organized individual to join our team as a Skin Cancer Care Coordinator. This is an exciting opportunity to make a real difference in the lives of patients affected by skin cancer.

Key Responsibilities
  • Provide comprehensive support and information to patients and families affected by skin cancer.
  • Assist the skin CNS Teams in the daily management and support of people affected by skin cancer regionally and nationally.
  • Work closely with clinical and administrative team members to ensure seamless care for patients.
  • Act as a conduit between patients and healthcare professionals to provide navigation and coordination of supportive care.
  • Develop and maintain relationships with local care providers, services, and sectors to promote a holistic and personalized approach to patient support.
Requirements
  • Relevant health or social care experience at Band 3 or equivalent.
  • Understanding of key referral pathways to Allied Health Professionals to support plans of care.
  • Experience in managing small projects and dealing with challenging situations.
  • Previous involvement with data collection systems and experience of working in a multidisciplinary team.
  • Excellent planning and organization skills, with the ability to work autonomously and prioritize workload.
  • Excellent communication, listening, and interpersonal skills, with the ability to demonstrate sensitivity and empathy.
