About This Role

We are seeking a dedicated and experienced Public Health Practitioner to join our Children's and Families Care Group within the Universal Services function. You will play a vital role in delivering the Healthy Child Programme to children, young people, and families across Stoke-on-Trent.

Your Responsibilities

  • Provide expert assessment of children and young people's care needs, developing and implementing tailored care plans.
  • Collaborate effectively with multidisciplinary teams and agencies to address a wide range of health and social care issues.
  • Contribute to the prevention of unnecessary hospital admissions and promote the overall well-being of the population served.
  • Supervise, train, and mentor junior staff members, fostering a supportive and collaborative team environment.
  • Demonstrate initiative and creativity in finding solutions to complex challenges within the healthcare system.
