Key Responsibilities- Establish and maintain positive relationships with service users, their families, and colleagues.
- Deliver evidence-based therapeutic interventions, in accordance with individual care plans, to support service users in achieving their recovery goals.
- Conduct physical health assessments, including vital signs and phlebotomy, to ensure comprehensive care.
- Contribute to the planning and delivery of therapeutic activities, promoting social inclusion, meaningful occupation, and independence.
- Support service users in accessing community resources and services, promoting their autonomy and well-being.
- Communicate effectively with the multidisciplinary team, ensuring seamless care coordination and collaboration.
- Participate in risk reviews and updates, ensuring the safety and well-being of service users.
- Work collaboratively with other Trust services to promote seamless care pathways and transitions.

To be successful in this role, you will need:
- NVQ 3 in (Mental) Health and Social Care, with demonstrable evidence of numeracy and literacy skills (equivalent GCSE's or Functional Skills).
- Experience working in mental health, with a focus on community-based care.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
