Job Summary

We are seeking a skilled Community Mental Health Support Worker to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care and support to individuals with mental health needs.

About the Role

The successful candidate will work under the direction of the key worker/multi-disciplinary team and deliver interventions to individuals with mental health needs. Your primary focus will be on assisting individuals to identify and achieve their agreed goals of recovery, including interventions related to meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living, and mental health recovery.

Key Responsibilities
  • Establish and maintain relationships with service users, relatives/carers, and colleagues.
  • Deliver agreed therapeutic interventions, in accordance with the care plan, to assist service users to achieve their recovery goals.
  • Undertake physical health assessments, including weight, height, blood pressure, ECG, and phlebotomy.
  • Contribute towards planning and delivering therapeutic activities with identified groups of service users as part of the care programme approach.
  • Support service users to access a range of activities and services to maintain their independence and promote their recovery.
  • Ensure that own actions support the equality, diversity, rights, and responsibilities of individuals and promote independence.
  • Regularly report and communicate service users' health status and care aims in care records and verbal reports to care coordinators and other members of the multidisciplinary team.
  • Have an awareness of community resources appropriate for service users to access.
  • Involvement in the review of risk, communicating any changes to the key workers.
  • Work with other Trust services to promote seamless service user-focused care, admission, transfer, and discharge on identified care pathways.
