About the Role

We are seeking a dedicated and experienced Senior Mental Health Practitioner to join our dynamic Home Treatment Team. In this role, you will play a crucial part in providing comprehensive care and support to individuals experiencing mental health challenges within their own homes.

Your Responsibilities

  • Conduct thorough clinical assessments to evaluate patients' needs and develop individualized care plans.
  • Provide evidence-based interventions and therapies, tailored to each patient's unique circumstances.
  • Collaborate effectively with a multidisciplinary team, including psychiatrists, nurses, social workers, and other healthcare professionals.
  • Liaise with families and carers to ensure their involvement in the care process and provide ongoing support.
  • Monitor patient progress and make adjustments to treatment plans as needed.
  • Maintain accurate and up-to-date records of patient care.
