Position Overview

Your Role: As a Youth Transition Support Specialist, you will oversee a diverse caseload that includes intricate cases, working autonomously and efficiently under the general guidance of a Team Manager. Your responsibilities will encompass:

  • Conducting initial adult care evaluations and reviews for individuals with care and support requirements, as well as their caregivers, in line with the principles of the Care Act 2014.
  • Developing care and support strategies that effectively address the needs of individuals requiring assistance and their caregivers.
  • Performing Mental Capacity evaluations and, when necessary, executing Best Interests Decisions in accordance with the Mental Capacity Act 2005.
  • Carrying out risk assessments and collaborating with individuals and the multidisciplinary team to implement well-considered positive risk management strategies.

Main Responsibilities

We are seeking a dedicated Youth Transition Support Specialist to become part of our Adult Social Care service, specifically within the Specialist services, including the Autism Team. This role focuses on assisting young individuals with apparent social care needs as they transition into adult services. As a social worker, you will be tasked with:

  • Conducting initial adult care assessments and reviews for autistic young individuals and their caregivers.
  • Collaborating with service users to formulate care and support plans tailored to their unique needs.

You will join a welcoming and highly supportive team of social work professionals, providing you with the opportunity to engage in a multidisciplinary role, collaborating with various professionals such as SEND Practitioners, Children’s Services, Psychologists, Occupational Therapists, Speech and Language Therapists, and educational staff.

About You

  • You must be a qualified Social Worker (holding a Social Work degree, Diploma in Social Work, CQSW, or equivalent).
  • Experience working with a diverse range of adults, including those with Autism, Mental Health Conditions, or Learning Disabilities is preferred.
  • You should be registered with Social Work England.
  • A Full UK Driver's License and access to a vehicle are required due to business needs.
