Youth Connect South West (YCSW) is a charitable organization dedicated to empowering young individuals in Bath and North East Somerset. Our mission is to offer a variety of services that assist young people in enjoying their present, preparing for their future, and realizing their aspirations.
We are looking for a committed and enthusiastic Youth Support Facilitator to become a vital part of our team. In this role, you will provide comprehensive support to young individuals aged 16-25 who have Special Educational Needs and Disabilities (SEND). Your primary focus will be to help them access educational resources and prepare for their transition into adulthood. This position entails creating tailored action plans, collaborating with various professionals, and guiding young people through their educational and personal growth journeys.
Key Responsibilities:
- Deliver personalized support to young individuals with SEND.
- Design customized action plans that highlight strengths, needs, and aspirations.
- Facilitate access to volunteering, work experience, and educational pathways.
- Serve as the main point of contact for young individuals and their families.
- Work collaboratively with a team of professionals to provide integrated support.
- Participate in and contribute to multi-agency meetings.
- Ensure high attendance rates and timely completion of assessments and action plans.
- Engage in one-on-one activities tailored to the unique needs of young individuals, including vocational, PSHE, and enrichment activities.
- Assist in planning next steps beyond the program, ensuring smooth transitions.
- Maintain precise records and adhere to safeguarding and data protection protocols.
