Role Overview

The position involves supporting the delivery of exceptional care and assistance to patients and their families, utilizing a person-centered methodology to enhance their health and recovery.

Key Responsibilities
  • Execute tasks as instructed by senior personnel and the multidisciplinary team, with appropriate guidance and oversight.
  • Facilitate the provision of high-quality care, ensuring the well-being and recovery of service users.
  • Adopt an empowering approach to assist service users in their personal recovery journeys.
  • Engage in the planning, execution, and assessment of therapeutic care programs for patients, including motivational activities.
  • Monitor and report any alterations in the condition or overall needs of service users.
  • Maintain high standards of verbal and written communication regarding patient care with all team members.
  • Foster and sustain positive relationships, communication, and collaboration across all disciplines.
  • Participate actively in the Care Programme Approach.
Standards and Compliance

Ensure that all responsibilities are performed to the highest standards in alignment with local and Trust policies and procedures.

Adhere to local and Trust regulations, including attending mandatory training and refresher courses.

Complete all administrative tasks promptly and efficiently.
